Green Local Schools

You are required to complete a background check before being hired and then every 5 years after that. The Ohio Department of Education (ODE) only accepts electronic copies. You MUST tell the agency performing the check that you need them sent to ODE. 

The location for the background checks is at the Tri County Educational Services Center, 741 Winkler Dr, Wooster, OH 44691.

Please ask them to send results to Green Local Schools Board office ℅ Courtney Knight.

Hours for Processing Background Checks: 8:30am – 4:00pm, Monday – Friday Requirements: State Issued Photo Identification or Passport and Social Security Number.

Form of Payment:  Currently there is not cost to Green Local School Coaches

Coaches Tool Chest is a once a year program that keeps our coaches up-to-date with all certification requirements. There is no cost to our coaches.  Please contact Mrs. Bohley in the SHS/GMS office for an access code.

• CPR Course (renewed every 2 years or yearly with CTC)
• First Aid Course (renewed every 3 years or yearly with CTC)
• Fundamentals of Coaching and Concussion Training (renewed yearly)
• Lindsay’s Law - Cardiac Arrest (renewed yearly)
• Mental Health Training (renewed yearly)
   

When you complete the course each year you should sync the completion date to your Final Forms account or email your certificate to the athletic department.

COACHES TOOL CHEST DIRECTIONS

You can apply for your PAP through your OH|ID account at the Ohio Department of Education. If you have never had an Ohio certifications before, you will need your drivers license to create your account. Here is the link to ODE. When you are searching for superintendent’s signature use our IRN #050559 to ensure your application is routed to Green Local Schools for approval. NEW: You must also upload your Coaches Tool Chest certificate to the Mental Health Component section of the application.

Volunteer Coaches must complete Coaches Tool Chest, a Background Check, and get a Pupil Activity Permit. Return all documents to Kellie Bohley in the Athletic Office. 

While extensive, these manuals can’t cover every particular situation. Reach out to your head coach and athletic director when you are unsure of how to proceed.

All fundraising efforts must be coordinated with our Smithville All-Sports Booster Club. 

Fundraising Request Form

Any group or organization that needs to use a facility, that is not directly connected to a school athletic program, you will need to fill out and turn in the district Facility Use Form.

Example: Parks and Recreation, Youth Sports, Outside team practices

For athletic facilities during regular use, you can just email Kellie Bohley.

FACILITY USE FORM

This Open gym/Field/Mat/Weight Room Waiver, Release and Assumption of Risk must be signed by any individual using the Green Local Schools District’s gymnasium and/or other facilities during times when such facilities are open to the public.

This is not for convenience. This is for legitimate safety reasons. 

RELEASE AND ASSUMPTION OF RISK WAIVER

Your form must be returned to athletics in enough time to get it board approved at least two weeks to the event.
